Documents needed for debt settlement
There will be two types of documents needed from you. First will be your monthly statements, and second will be documents filled out that we send you.
- The most recent monthly statement for each account that you want to enroll in our debt settlement program. Each account must be higher than $700 to be included and your total debt must exceed $10,000.

- Client Personal Information Sheet (print out, fill out and fax or email us)
- Limited Power of Attorney (1 page - for us to contact your creditors)
- Client Creditor Worksheet (2 pages - debts and hardship explained)
- Debt Settlement Agreement (5 pages - our debt settlement agreement)
- Electronic Payment Authorization (1 page - set-up your auto-payments)
